- Recognize the risk of hospitalization and mortality associated with heart failure (HF) even in the setting of mild symptoms and evidence-based treatment
- Apply key learnings from the 2017 Comprehensive Update of the Canadian Cardiovascular Society Guidelines for the Management of Heart Failure into their practice
- Assess opportunities for family physicians to optimize patient care across all phases of the HF syndrome trajectory
- Recognize key signs that trigger referral of patients to specialist HF care
- Evaluate the role of novel therapeutic options and their optimal role in the management of HF
