Course Description

Each year an increasing number of people worldwide are being diagnosed with diabetes with potentially devastating complications.
Although the majority of patients with type 2 diabetes are diagnosed at the primary care level, there are a significant portion of patients with undiagnosed hyperglycemia who may present at specialist care for complications or other conditions. Endocrinologists, cardiologists, and nephrologists must collaborate with each other and primary care clinicians to ensure that continuity of
diabetes care is maintained with all these patients.

  1. Determine the clinical considerations of managing a patient who was diagnosed with type 2
    diabetes when having a stroke
  2. Initiate appropriate antihyperglycemic therapy to manage a patient’s diabetes and further reduces their risk of a secondary major adverse cardiovascular event (MACE)
  3. Review the steps to managing a patient with mental health disorder who is nonadherent to their
    treatment plan
  4. Collaborate with other specialists and primary care clinicians to review how the patient’s care
    team can optimize diabetes management and its complications
