When patients are admitted to hospital, the management of their diabetes can be very different than in the community. The types and doses of medications used for diabetes management may differ from
patients’ usual regimens, and glycemic control may be one of many acute medical issues being addressed. It is critical that medication regimens be reviewed and optimized prior to discharge.
