Upon completion of this program, participants will be able to:

• Identify patients in your practice who are at highest risk of pneumococcal disease.
• Assess vaccination status for adult patients.
• Reduce the risk of pneumococcal disease in your practice by offering eligible patients guideline-recommended vaccines.
