But clinical reality shows us that if a patient hasn&#8217;t had a meaningful response by then, that specific medication is unlikely to suddenly start working.<\/p>\n\n\n\n<p>So, what\u2019s actually happening? Traditionally, we\u2019ve called this <strong>treatment-resistant depression (TRD) <\/strong>\u2014 defined as <a href=\"https:\/\/www.mdbriefcase.com\/fr\/cours\/navigating-non-response-strategies-to-address-poor-antidepressant-response\/\">failing two or more adequate trials<\/a>. However, many clinicians are moving toward the term <strong>difficult-to-treat depression.<\/strong> It\u2019s a bit more compassionate and acknowledges that the challenge lies in the complexity of the disease, not a &#8220;resistance&#8221; in the patient.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Why the poor response?<\/strong><\/h3>\n\n\n\n<p>When a treatment stalls, it\u2019s time to troubleshoot. Before reaching for a new script, consider these common hurdles:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Is the diagnosis correct?<\/strong> Depression is often secondary to other issues. Are you looking at underlying Bipolar Disorder, ADHD, or a medical issue like thyroid dysfunction? If the diagnosis is off, the treatment will be too.<\/li>\n\n\n\n<li><strong>The impact of previous trials:<\/strong> If a patient has already tried three different antidepressants without success, <a href=\"https:\/\/www.mdbriefcase.com\/fr\/cours\/navigating-non-response-strategies-to-address-poor-antidepressant-response\/\">the odds that a fourth or fifth<\/a> trial will work are statistically low. These patients likely need alternative strategies or adjunctive therapies.<\/li>\n\n\n\n<li><strong>The trauma factor:<\/strong> This is a big one. For patients with a history of trauma, medication alone is rarely enough. Without integrating psychotherapy to tackle the underlying trauma, the &#8220;biological&#8221; fix often falls short.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Basics first<\/strong><\/h3>\n\n\n\n<p>It sounds simple, but it\u2019s easy to overlook: did the patient reach a full therapeutic dose? And did they stay on it long enough?
