Over the last few decades, the incidence rates of cervical cancer\u00a0<\/span><a href=\"https:\/\/www.canada.ca\/en\/public-health\/services\/chronic-diseases\/cancer\/cervical-cancer-facts-figures.html\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">have decreased steadily<\/span><\/a><span style=\"font-weight: 400;\">, with increased awareness, screening, and testing the likely reason.<\/span><\/p>\n<h2><b>Cervical Cancer Research Update<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Cervical cancer research has already led to several breakthroughs in prevention and treatment.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The American Association for Cancer Research\u00a0<\/span><a href=\"https:\/\/www.aacr.org\/blog\/2021\/01\/07\/cervical-cancer-advances-in-prevention-screening-and-treatment\/\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">reports<\/span><\/a><span style=\"font-weight: 400;\"> that various new immunotherapeutic strategies (for example, the recently\u00a0<\/span><a href=\"https:\/\/www.fda.gov\/drugs\/resources-information-approved-drugs\/fda-approves-pembrolizumab-advanced-cervical-cancer-disease-progression-during-or-after-chemotherapy\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">approved<\/span><\/a><span style=\"font-weight: 400;\"> checkpoint inhibitor pembrolizumab [Keytruda]) are being used in the successful treatment of cervical cancer.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A\u00a0<\/span><i><span style=\"font-weight: 400;\">Lancet Oncology\u00a0<\/span><\/i><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/30795950\/\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">modeling study<\/span><\/a><span style=\"font-weight: 400;\"> also indicates that the continued spread of cervical cancer screening and availability of HPV vaccines could prevent approximately 13 million global cases of cervical cancer by 2069.<\/span><\/p>\n<h2><b>Screening for Cervical Cancer<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Regular cervical cancer screenings are paramount for early detection, leading to the best preventative and treatment efforts. According to the\u00a0<\/span><a href=\"https:\/\/cancer.ca\/en\/cancer-information\/cancer-types\/cervical\/screening\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">Canadian Cancer Society<\/span><\/a><span style=\"font-weight: 400;\">, there are two screening options patients should be aware of:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>The pap test:\u00a0<\/b><span style=\"font-weight: 400;\">Pap tests (pap smears) screen for precancers, cells that might become cancers if left untreated. The CCS recommends that women receive a pap test every one to three years, starting at age 21.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>The human papillomavirus (HPV) test:\u00a0<\/b><span style=\"font-weight: 400;\">HPV is one of the\u00a0<\/span><a href=\"https:\/\/www.canada.ca\/en\/public-health\/services\/diseases\/human-papillomavirus-hpv.html\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">most common<\/span><\/a><span style=\"font-weight: 400;\"> sexually transmitted infections in Canada and a\u00a0<\/span><a href=\"https:\/\/hpvcentre.net\/statistics\/reports\/CAN_FS.pdf\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">leading cause<\/span><\/a><span style=\"font-weight: 400;\"> of various cancers, including anal, vaginal, vulvar, laryngeal, and oropharyngeal. Around\u00a0<\/span><a href=\"https:\/\/www.sac-isc.gc.ca\/eng\/1572286220756\/1572289813916\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">3 out of 4<\/span><\/a><span style=\"font-weight: 400;\"> sexually active Canadians are likely to contract HPV once in their lifetime. Nearly all cases of cervical cancer are believed to be linked to the HPV virus. HPV screening starts around 30, and women should speak to their gynecologists to determine the recommended testing frequency.<\/span><\/li>\n<\/ul>\n<h2><b>Prevention and Remedies<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">The best preventative remedy for cervical cancer is the HPV vaccine.<\/span><\/p>\n<p><a href=\"https:\/\/www.canada.ca\/en\/public-health\/services\/publications\/healthy-living\/canadian-immunization-guide-part-4-active-vaccines\/page-9-human-papillomavirus-vaccine.html\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">Health Canada recommends<\/span><\/a><span style=\"font-weight: 400;\"> that girls and women between the ages of 9 and 27 receive the vaccine. It may also be administered to those 27 and older who have an ongoing risk of exposure.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The Canadian government\u00a0<\/span><a href=\"https:\/\/www.canada.ca\/en\/public-health\/services\/diseases\/human-papillomavirus-hpv.html\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">has approved<\/span><\/a><span style=\"font-weight: 400;\"> three HPV vaccines:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Gardasil, 4vHPV<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Gardasil 9, 9vHPV<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cervarix, 2vHPV<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">It is also important to remind patients to continue getting regular screenings, even if they have received the vaccine.<\/span><\/p>\n<h2><b>Barriers and Challenges<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Despite increased awareness and access to screening in recent years, various barriers to testing continue to prevent thousands of women from being screened for cervical cancer.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A recent\u00a0<\/span><a href=\"https:\/\/bmcwomenshealth.biomedcentral.com\/articles\/10.1186\/s12905-021-01264-x\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">systematic review<\/span><\/a><span style=\"font-weight: 400;\"> of various studies revealed three primary categories of barriers and challenges to cervical cancer screening (CCS):<\/span><\/p>\n<h3><b>1. Awareness Barriers<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">72.2 percent of reviewed studies reported that lack of knowledge or awareness of cervical cancer screening\/prevention was a leading barrier to women opting for regular tests. Lack of physician recommendation and participants\u2019 health literacy gaps are contributing factors.<\/span><\/p>\n<h3><b>2. Socio-Cultural and Psychological Barriers<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Another common set of barriers to regular CCS is socio-cultural and psychological. A large number of young people appear resistant to testing due to factors like:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Anxiety<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fear of pain<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Embarrassment<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Sexual stigmas<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">General discomfort around visiting a gynecologist<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Additionally, &#8220;the fear of finding cancer&#8221; is another\u00a0<\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5494033\/\" target=\"_blank\" rel=\"nofollow noopener\"><span style=\"font-weight: 400;\">major psychological barrier<\/span><\/a><span style=\"font-weight: 400;\"> in this group.<\/span><\/p>\n<h3><b>3. Systemic\/Practical Barriers<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Systemic, economic, and practical barriers are the final category of reasons why young women ignore or delay testing.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The cost of testing is a significant factor, especially among women without healthcare insurance.
