    • la prise des antécédents pharmaceutiques les plus exacts possible en suivant un processus systématique
    • l’établissement de BCM lors de l’admission, du transfert et du congé
    • les rôles des professionnels de la santé, du patient et des membres de la famille dans la mise en œuvre d’un BCM
  4. Énoncer les obstacles à la réalisation d’un BCM et les facteurs qui en facilitent la création.
