Patients with diabetes will many times have multiple comorbid conditions that will require each specialist to collaborate to prioritize the care. Endocrinologists, cardiologists, and nephrologists must collaborate with each other and primary care clinicians to ensure that continuity of diabetes care is maintained with all these patients. This program will review the management of a patient case with glycemic, cardiovascular, and renal issues.

Learning objectives

Upon successful completion of this continuing education module, the participant will be better able to:

  1. Determine the management of hyperglycaemia in a patient with heart failure and reduced renal function.
  2. Review the considerations of starting an SGLT2 inhibitor in a patient with heart failure.
  3. List the management strategies of hyperkalaemia in a patient with chronic kidney disease and heart failure.
