Après avoir complété avec succès cette formation continue, le pharmacien sera mieux outillé pour :

  1. Définir le Bilan comparatif des médicaments (BCM);
  2. Discuter des avantages du BCM;
  3. Mettre en œuvre un processus systématique durant une consultation avec le patient afin de recueillir un Meilleur schéma thérapeutique possible (MSTP);
  4. Connaître les différentes sources d’information au sujet des médicaments;
  5. Expliquer les processus de BCM au moment de l’admission à l’hôpital et lors du congé de l’hôpital;
  6. Discuter de l’importance de la documentation et de la communication lorsque le patent transite à l’intérieur du système de santé;
  7. Comprendre le rôle que peut jouer le personnel de la pharmacie pour assurer la sécurité du patient lors des transitions de soins.
